Cardiac Home Follow-Up Program

Condition Specific Support

Provided by Vancouver Coastal Health

Provides in-home follow-up care for patients who have been admitted to the Lions Gate Hospital Cardiac Unit.
Assist individuals and their family members who have experienced a myocardial infarction and/or coronary artery bypass graft surgery to achieve optimal physiological, psychosocial, and vocational status.
